Unified Multi-Purpose ID (UMID) Card: What You Need to Know
The Unified Multi-Purpose Identification (UMID) card is a valid ID that provides a single identification card for all members of the Social Security System (SSS), Government Service Insurance System (GSIS), Home Development Mutual Fund (HDMF or Pag-IBIG Fund), and the Philippine Health Insurance Corporation (PhilHealth) in the Philippines. This card was introduced in 2011, replacing the old SSS ID, and is considered the de facto SSS ID.
What is a UMID ID Card Used For?
The UMID card serves as a valid ID for government offices and other entities in the Philippines. Members of the SSS and GSIS can choose between a regular UMID card or a UMID-ATM card. The UMID-ATM card can also be used to withdraw SSS or GSIS benefits and loans. Regular UMID cardholders can upgrade to the UMID-ATM by applying for a card replacement at any SSS branch nearby. SSS members can also use the ID to view contributions, check loan status and balance, and see other information at SSS branches.
Who is Eligible for a UMID Card?
The following conditions make an individual eligible for a new or replacement UMID card:
- Being an SSS member with at least one posted contribution
- Lost, stolen, or damaged SSS ID or UMID card
- Updating UMID card due to a change of name or making corrections in personal data
- Upgrading regular UMID card to UMID-ATM card
- Replacing an old SSS ID with a UMID card
What are the Requirements for a UMID Card Application?
If you want to apply for a UMID Card, you are required to submit one valid primary ID, or two valid secondary IDs if no primary ID is available, when applying for a UMID card. Both primary and secondary IDs must be original copies. Here are the valid primary and secondary IDs:
Primary Valid ID for UMID:
- Unified Multi-Purpose ID (UMID) Card (for replacement or upgrading)
- Social Security Card
- Passport
- Driver’s License
- Postal Identity Card
- Voter’s ID Card
- National Bureau of Investigation (NBI) Clearance
- Seafarer’s Identification and Record Book (Seaman’s Book)
- Alien Certificate of Registration
- Firearm Registration
- License to Own and Possess Firearms
- Permit to Carry Firearms Outside of Residence
Secondary Valid ID for UMID:
If you don’t have any primary IDs listed above, you can use the following secondary valid IDs. Submit two secondary valid IDs, both must have signatures, and one of the following IDs should have a photo of you:
- School or Company ID
- Senior Citizen Card
- Taxpayer’s Identification Number (TIN) ID
- Pag-IBIG Member’s Data Form (MDF)
- PhilHealth Member’s Data Record (MDR)
- Birth Certificate
- Marriage Contract
- Police Clearance
- Overseas Workers Welfare Administration (OWWA) Card
- Baptismal Certificate
- Transcript of School Records
- Permanent Residency ID
- Health or Medical Card
- Bank Account Passbook
- Credit Card
- ATM Card with Cardholder’s Name
- Life Insurance Policy
- ID Card issued by the LGU (Barangay/Municipal/City)
- ID Card issued by a professional association recognized by the Professional Regulation Commission (PRC)
- Fisherman’s Card issued by the Bureau of Fisheries and Aquatic Resources (BFAR)
- Temporary License issued by the Land Transportation Office (LTO)
- Seafarer’s Registration Certificate issued by the Philippine Overseas Employment Administration (POEA)
- Certificate of Naturalization from the Bureau of Immigration
- Certificate from the Office of Muslim Affairs
- Certificate from the Office of Northern/Southern Cultural Communities
What are the Ways to Apply for a UMID card?
You can apply for a UMID Card with SSS in two ways:
Online Appointment System. You may set an appointment via your My.SSS at your preferred SSS branch. This option is best if your SSS branch doesn’t allow walk-ins.
Branch Walk-In. You may go to your nearest SSS branch with a UMID enrollment facility. If you prefer this method, follow the mandatory Branch number coding system implemented by SSS:
10th Digest of SSS Employer (ER) ID Number – Appointment Day
1 and 2 – Monday
3 and 4 – Tuesday
5 and 6- Wednesday
7 and 8 – Thursday
9 and 0 – Friday
This number coding system is implemented in all SSS branches in Metro Manila, and other branches including Baguio, Bacolod, Bacoor, Biñan, Cagayan de Oro, Cebu City, Dagupan, Davao, Lapu-Lapu City, and Iloilo-Central.

If you applied for an SS number online starting December 10, 2020, take note of the following:
- No need to fill out the UMID Application Form
- Schedule an appointment for biometric data capture via the My.SSS Appointment System.
- Present your printed SSS Number Application and transaction number slip on your appointment date at the SSS branch you chose.
How Do I Apply For a UMID Card?
Once you have determined that you are eligible and have the necessary requirements, you can start the UMID Card Application. Here are the steps on how to get a UMID Card:

Prepare your valid primary or secondary ID.
Make sure to bring your original primary or secondary ID and a photocopy of it when you go to the SSS branch. Ensure that your ID is valid and not expired.

Fill out the UMID card application form.
The UMID card application form can be downloaded from the SSS website or obtained at the nearest SSS branch. You must fill out the form completely and accurately using a black or blue ballpen.
Write in capital letters and write “N/A” if the information is not applicable to you. Take note that only up to two erasures are allowed on the form, and you must affix your signature on each erasure or alteration.

Set an Online Appointment with SSS
Set an appointment online by going to the My.SSS portal and logging in to your account. Click “Member Info” on the menubar and click “Appointment System.” Fill in the required information, and choose the time for your appointment. After completing everything, click “Submit Schedule” to book your appointment.

Go to your Nearest SSS Branch on the Scheduled Appointment
Once accomplished, bring the UMID application form, your valid primary or secondary ID, and the photocopy of your ID to the SSS branch at least 15 minutes before your scheduled appointment. Submit your requirements to the designated personnel at the UMID enrollment facility.

Have your photo and biometrics taken.
Once your requirements have been verified, you will have your photo and biometric data taken at the enrollment station. The process involves taking your facial image, signature, and fingerprints.
Wear proper attire on the day of your appointment. It is recommended to wear collared shirts or blouses. Your face and neck should not have any bandages and accessories, such as necklaces, earrings, metal piercings, or hats. Also, take off your eyeglasses or sunglasses before taking your photo.
However, you need to wear your face mask to get a UMID Card for safety protocols. You will only remove them when taking your photo.
Once you’re done taking your photo and capturing your biometrics, an SSS personnel will detach the acknowledgment stub at the bottom of the UMID card application form.

Check the Status of Your UMID Card Application
After completing the enrollment process, you will be informed when your UMID card is ready for delivery. You can also check the status of your UMID card through the SSS website.
To check the status of your UMID Card, you may contact the SSS hotline at 8920-6401 local 5714 or email [email protected]. You can ask for the availability to get UMID card if you did not get a text from SSS within 30 days after your application.
You can also check online through the My.SSS portal:
- Log in to the My.SSS portal
- Select “Member Info”
- Click “UMID/SSS ID Details” from the drop-down option
- You will see the details of your SSS ID or UMID Card, such as the enrollment date, serial or transaction number, and transaction status.

Claim Your UMID Card
You will receive a text message from SSS that your UMID Card is ready for pick-up. You may claim your card at the sss branch where you made your application within 60 days of receiving the text message. Note that all unclaimed UMID Cards will be destroyed after five (5) days.
